2017 Online Application Form for REDSHIRTS ONLY



Please now read the following guidance notes, so that you enter the correct information when asked.


Please complete this Application Form for each individual Redshirt. Please note, your application cannot be processed unless you confirm, when asked on the application form, that you have read and agreed to the Code of Conduct, which can be accessed by clicking here. It may be worth printing these instructions before you start, so that you can refer to them when filling out the form.


The closing date for applications is 24th March 2017. Please apply early to avoid disappointment. Any amendment or cancellation of this booking has to be in writing to A&B Lourdes Pilgrimage Office - The Redshirts, The Bothy, Cowfold Road, Coolham, RH13 8QL or by email - click here

THE COST FOR THIS YEARS PILGRIMAGE is £599
(Cost inclusive of Transport, Accommodation and All Meals)

Payments can be made in instalments, please indicate on the application if you require a standing order form.


Final balance due by 31st May 2017 unless a payment plan has been agreed with the Pilgrimage office. (Final balance for payment plans to be completed by 31 August 2017)



APPLICATION DETAILS

Before starting your application, please make sure you have the following documents with you:

✔ Your European Health Insurance Card ("EHIC Card")
✔ GP's contact details, and a list of your medication (if applicable)
✔ A photograph of yourself, that you can either post, A&B Lourdes Pilgrimage Office, The Bothy, Cowfold Road, Coolham, RH13 8QL or send by email click here


FINANCIAL INFORMATION

Please indicate if you wish to pay the balance by instalments. Having agreed a payment plan with the Pilgrimage Office a form will be sent to you or your parent/guardian to complete. Final payment of the instalments must be paid by 31 August 2017.



FURTHER INFORMATION PACK

Once we receive your completed application, we will send you a further pack of information for you to complete and return. A copy of your complete application will be sent to you for your parent/guardian to sign and return. A short questionnaire regarding why you wish to become a Redshirt will be included, you will also be asked for the contact details of two referees. One of your referees must be your school Head of Year or Personal Tutor, the other should be a Parish referee (for example your Parish Priest, Confirmation Catechist, Youth Group leader etc). The Pilgrimage Office will contact the referees on your behalf for a reference, it would be helpful if you could ask these referees for their permission and contact details in advance so as not to delay the progress of your application.



SAFEGUARDING

In line with the Diocesan Safeguarding Policy, ALL REDSHIRT GROUP LEADERS, (PLUS ALL OTHER HELPERS, DOCTORS, NURSES AND CHAPLAINS) will have an Enhanced DBS Disclosure Certificate.



TRAVEL INSURANCE

All Redshirts are included in a group group Travel Insurance Policy held by the Pilgrimage Trust. The cost is included in the quoted price. A copy of the Travel Insurance Policy will be sent with your confirmation of booking. Click here to view the insurance policy.



MEDICAL INFORMATION

You will be asked to complete the Confidential Medical Questionnaire. As our Doctors will support all Redshirts whilst we are in Lourdes, it is important that you provide details of your medical condition(s) and treatment. We will not be able to accept this application if the Medical Questionnaire has not been completed.



EUROPEAN HEALTH INSURANCE CARD (EHIC)

ALL REDSHIRTS need an EHIC which will be required in the event of a pilgrim needing medical attention in Lourdes. The EHIC (which is free of charge) can be obtained online at www.ehic.org.uk or phone 0300 330 1350. Please note that an EHIC must be obtained to validate your travel insurance.



PASSPORTS/VISAS

If you are a non-British/EU citizen you may need a visa to travel to France and possibly specific permission to re-enter the UK. Please check with your embassy and the French embassy to be sure about what you need. It is your responsibility to ensure you have valid travel documents.



HEALTH & SAFETY

The Trust has a Health & Safety Policy. All Redshirts are expected to follow any health and safety procedures laid down by the organisation and behave in a responsible manner for their own welfare and the welfare of others. A copy of the policy is available from the Pilgrimage Office on request.



FINAL TRAVEL INSTRUCTIONS

Complete travel details and final pilgrimage instructions will be sent approximately two weeks before the day of departure. These will only be sent if full payment has been received by the 31st May or a payment plan is in place.



DATA PROTECTION ACT

Under the Data Protection Act personal information from the booking form will be registered on a computerised database for processing applications.
